Are you an experienced caregiver with a passion for supporting individuals facing challenging circumstances? Your Choice is seeking a dedicated caregiver with expertise in crisis management, de-escalation, and restraints to provide comprehensive support to manage an individual dually diagnosed with a developmental disability and mental illness.

The individual is a remarkable person with a diagnosis of PTSD, which presents a multitude of challenges requiring specialized care. As this individual’s caregiver, you will play a crucial role in ensuring their health, safety, and well-being, particularly during moments of behavioral crisis.

We are looking for full-time and part-time caregivers to work 2-4 shifts per week, including weekends. We offer day and night shift positions with competitive compensation.
• Day Shift: 4 pm - 12 am
• Night Shift: 12 am – 8 am
• Weekend Shift: Flexible 8 hours shifts available during the day and overnights

• Provide line of sight supervision at all times during shift.
• Provide verbal prompting and supervision in all aspects of daily living, including bathing, dressing, toileting, mobility, and eating, with an emphasis on de-escalation techniques during challenging behavioral moments.
• Assist with medication management, ensuring proper administration and monitoring for side effects.
• Accompany the individual alongside additional caregivers during outings in the community.
• Coordinate and accompany the individual to appointments, providing support during consultations and ensuring follow-up care, with a focus on crisis intervention if necessary.
• Implement behavioral management strategies recommended by therapists to address mood fluctuations and anxiety, particularly during times of heightened behavioral distress.
• Collaborate with the individual’s support team to ensure continuity of care and adherence to treatment plans.
• Maintain open communication with the individual’s family and other support providers to address any concerns or changes in their condition.

• Previous experience working with individuals with complex mental health and behavioral health needs, particularly in crisis management, de-escalation, and restraints, with knowledge of developmental disabilities, dual diagnoses, and associated behavioral challenges.
• Training in crisis intervention techniques, restraints, and/or behavioral management.
• Strong communication and interpersonal skills, with the ability to provide compassionate care and build trusting relationships.
• Patience, empathy, and a positive attitude when supporting individuals with complex mental health needs.
• Background check completed after initial interview.
• 18 years or older.
• Ability to work independently and as part of a collaborative team to meet the individual's needs effectively.
• Ability to be QMAP certified, if not already.
