Duties and Responsibilities:
Hours: Assisted Living Home is in need of a caregiver to work 1-4 shifts a week. Shifts are 7am-7am
1. Assist resident with activities of daily living as needed in resident’s care plan.
2. Administer medications as directed in resident’s care plan.
3. Preparation and serving of meals.
4. Assist with housekeeping and laundry services.
5. Provide all services according to resident’s Care Plan.
6. Demonstrate knowledge of standard precautions and infection control.
7. Demonstrate knowledge of and adhere to procedure for fire, life safety, disaster, work safety and other emergency procedures.
8. Assist with activities.
9. Complete personal care including baths, back rubs, oral hygiene, shampoos, and to change bed linens as often as necessary.
10. Assist in dressing and undressing residents.
11. Report on resident's condition and significant changes to the Administrator.
12. Adhere to company policies and procedures.
13. Perform other duties as assigned.
Duties:
- Develop/implement a conditioning/rehabilitation program consistent with physician's Plan of Treatment and the overall goals of the patient/rehab team. Adjust treatment as needed to achieve maximum results.
- Confer with physician and other team members to obtain additional patient information and assist in developing, implementing, and revising the therapy treatment program and Plan of Treatment.
- Coordinate, oversee, and evaluate. OTR to implement treatment protocols and collect data on patient progress as permitted by policy and regulation.
Required skills:

- Experience with a high volume of rehab patients in Home Health, outpatient or in-patient settings
